What we’ll ask you during enrolment
To support a smooth and informed onboarding process, we may ask for the following information:
Participant Details
- Full name
- Date of birth
- Home address
- Parent, guardian, or nominee details (if applicable)
- Preferred contact information
NDIS Information
- NDIS number
- Plan start and end dates
- Plan management type
- (Self-managed, Plan-managed, or NDIA-managed)
- Relevant funding categories
Support Information
- Participant goals
- Type of supports being requested
- Preferred days and times
- Support setting
- (in-home, community, or future centre-based programs)
Health, Safety & Wellbeing
To ensure safe and responsive support, we may ask about:
- Medical conditions
- Allergies
- Medications
- Behavioural or sensory considerations
- Communication preferences
- Mobility or personal care needs
- Supervision or safety requirements
Providing documentation can help us better understand and support each participant:
- NDIS plan or plan summary
- Therapy or specialist reports
- Behaviour support plans or risk assessments
- Medical or emergency management plans
